If the request is made by an agent, please enclose the following documents in addition to the above documents.
(1) In the case of a legal representative (person with parental authority or adult guardian), one of the following documents as proof of legal representative.
1. A copy of the family register 2. A copy of the health insurance card with the dependents entered 3. A certificate of registered matters
(2) In the case of a proxy, a letter of attorney (with the actual seal of the proxy) and a certificate of the proxy’s seal impression.
(3)Identification document of the agent (If your permanent address is written on it, please paint it black.)

For requests of disclosure, we charge a fee of JPY1,000 per application, so please enclose a postal money order of JPY1,000 in the application. *If the fee is insufficient or not enclosed, we will inform you, however if you do not pay within the specified period, we will assume that you did not request disclosure.
Purposes of use of personal information obtained through requests for disclosure, etc.
Personal information obtained in connection with a request for disclosure shall be handled only to the extent necessary to respond to the request for disclosure.
